Blood pressure monitoring
If you are able to take blood pressure readings at home, please submit your readings using this form.
Cervical Smear Test
Women aged between 25 and 49 years are invited to have a cervical smear test every 3 years and women aged between 50 and 64 will be invited to have a smear test every 5 years.
You will receive booking instructions with your invitation.
Contraceptive pill review
If you have been asked to complete a contraceptive pill review form, please do so here. The form will be reviewed by our specialist nurse who will contact you if necessary.

Walk-in Blood Tests
We operate a walk-in blood test clinic every Monday to Friday between 8am and 10am (excluding Bank Holidays). These clinics are for our adult (over 16) patients only.
