Spring 2025 Covid Vaccination Campaign Invitation Form

From April 2025 the practice will be offering eligible patients a covid vaccination as part of the Spring Booster 2025 Campaign.

Eligible cohorts for the spring 2025 programme are:

• adults aged 75 years and over
• residents in a care home for older adults
• individuals aged 6 months and over who are immunosuppressed

If you are eligible, the practice will contact you with information on upcoming clinics and how to book. 

If you feel that you should have been invited but haven't received an invitation, please complete the following form.
We will review your medical record and if we are able to offer you a vaccination under the eligibility criteria, we will send you an invitation to book an appointment.  If, after reviewing your medical record, you do not meet the criteria we will advise you of that.
