Our Health Support Workers provide more specialist support to patients in areas of mental health and wellbeing
Community Connect Social Prescribing Link Worker
Community Connect is a Social Prescribing service for Cherwell and West Oxfordshire. We have our own dedicated Social Prescriber to support our patients.
How can they help? The Social Prescriber focuses on ‘what matters to you’ and take a holistic approach to people’s health and wellbeing. She connects people to community groups and services for practical and emotional support. The service is free of charge and she can work with you for up to 12 weeks to help you achieve your goals.
Some examples of how our Social Prescriber can help are:
- Increasing social connections – connect you to befriending schemes or meet new people by joining in with local groups like art classes or gardening
- Money and budgeting – help you to get advice about entitlement to benefits, money management and what to do about debt
- Gain confidence and skills – find training to help build your confidence and skills or move into work and volunteering
- Healthy lifestyle – become more active and develop a healthier lifestyle, which can help manage long term health conditions

Oxfordshire MIND Wellbeing Workers
- First point of call in triaging for patients with mental health or wellbeing concerns
- Can offer up to 6 wellbeing sessions with OxMind
- Offer signposting and self-help advice for people to manage their wellbeing
- A safe space to talk
- Non-clinical medication reviews and mental health reviews
Physiotherapists
Our Physiotherapists undertake initial patient reviews and from this may offer advice or onward referral.
