Registering with the Practice
We welcome new patients to the Practice and you can register with us using the link below or, if you prefer, by collecting a paper form from the surgery to complete.

The link will check that your address is within our Patient Boundary Map and are eligible to register with us.
Once you have completed the registration you will need to come into the Practice with one proof of ID and one proof of address to complete your registration (Driving Licence, Passport, Utility bill etc). For children we will need a copy of their birth certificate or passport so we can verify their name and date of birth.
Please allow 5 working days for us to process your registration before making a routine appointment – if you have an urgent issue, then please contact us. We will register you for Patient Online Services, and you will be able to get your log on details from our team.
We ask all new eligible patients to attend for an NHS Health Check appointment to establish your current state of health, check your blood pressure and offer further services as appropriate. Once registered, we will call you to book an appointment.
If you have moved from another part of England or Wales, then your medical records will be electronically transferred to us along with any paper records. Patients moving to us from outside of England or Wales are asked to obtain a paper copy of their medical records, which can then be saved to your new medical record.
Leaving the Practice or Moving Out of the Practice Patient Area
We maintain a secondary patient area known as the “Grey Zone” around the outside of our main patient area, which allows current patients who have relocated there to continue to be a registered patient at the Practice.

If a patient moves outside of this Grey Zone boundary then the Practice Policy is that patients are required to transfer to a practice that covers their new address. This is because we are unable to provide the appropriate care to patients living at distance from us for which we are rsponsible for; in addition with the growth of Bicester we do not have the capacity to maintain patients living at distance.
By registering with a new Practice the patient’s records are automatically transferred to them from us electronically. Any paper records are also transferred via the NHS system in place across England. If moving out of England and Wales, then the patient records are transferred to the NHS for safekeeping.
Where a patient has moved and have not registered with a new practice, then after 30 days they will automatically be deregistered and their medical records transferred to the NHS for safekeeping. Please note that once the patient has moved outside of our area then we are unable to provide any Home Visiting Service, so the only reliable way to secure access to this service is to reregister promptly with another local GP service.
Patient Conduct
We appreciate that the vast majority of patients are a pleasure to have at the Practice. But in maintaning a safe and welcoming space for all staff and patients to be in, the Practice has a zero tolerance to patient misconduct.
Such misconduct can be viewed as a breakdown in the Patient – Practice relationship and therefore result in a patient being deregistered from the Practice. Where violent behaviour is shown then the patient will be deregistered with immediate effect and the situation reported to the NHS and also the Police as appropriate.
